1. A deposit of $100 or more made out to Patricia Kortekaas, PT (Class fee is $750 - fees increase to $850 in 2013.)
Retakes must occur within two years for a discount.

Introduction, Advance: $400. After two years: $600
Extremities, Cranial, Vascular: $600. After two years: $750
